He's holding a lute in his left hand, but it's not actually equipped on his person. Should there be a note in his equipment section or should it just be left out all together?

I think it'll be fine, since a lot of NPCs in-game hold in-game items, but not in the natural fashion. It's obvious enough that he hasn't actually equipped the lute, at least to me. Plus it really isn't all that important anyway.
