How to organize?

It came to me that is may be a good idea to organize the effects with the equipment based instead of effects. (Much like JP wiki as organized) You are likely to look at what effects are available for certain equipment and choose what best suits you from the available effects.(at least when the additional toolkits are released and you are able to re-reforge your items.)

Another reason that this is more effective, is that some effects have different cap level on different equipments. For example, Smash damage goes as high as 20 on two-handed swords while only 10 for one-handed weapons. Rather then making note of that, it is better to simply create an item based chart to avoid those issues.

That sounds like a good idea.

I'm starting to think that it'd be good to mention these reforges on the skill pages... and perhaps on equipment pages as well. That latter part will be much easier for reforges that are only applicable to a couple of items.
