How should in-game Title errors be handled?

Should a section of the wiki page be devoted to them? I've not messed with Titles itself, but on other Category:Titles pages (so far being Master Titles and Numbered Titles), I've been deleting the messages denoting an error and correcting the title effect itself.

I'm fine so long as it mentions the error somewhere in the entry itself, not like a note at the very bottom of the page unless you add a ref.
How does one code a Ref tag?
